Adopt-a-Seat Fundraiser Request Form

The Parents in Support of the Arts (SPSA) is helping the South Glens Falls Music Department purchase a baby grand piano for the newly renovated High School Auditorium.  We have started an "Adopt-a-Seat" program to let community members "adopt" an auditorium seat for a fee of $50 per individual or family and $100 for local businesses.  By adopting a seat, participants' names or a short message will be engraved on a metal nameplate to be permanently affixed to their adopted seat.  Adopt a Seat contributions are tax-deductible.

If you are interested in adopting a seat for the High School Auditorium, please write a check made out to SPSA and return it with this completed form to: SPSA, c/o Dolores Bullivant, South Glens Falls High School, 42 Merritt Road, South Glens Falls, NY 12803.  We will contact you when your payment is received and also let you know when your nameplate has been attached to your adopted seat.

Yes, I would like to adopt an auditorium seat in the South Glens Falls Auditorium.  Attached is my check made out to SPSA in the amount of:

$50 - For a Family or Individual 
$100 - For a Business or Association 

There is a maximum of three lines for your custom message.  The first line has 28 characters available (including spaces), the 2nd has 20 and the third has 28.  Some examples would be: "In Memory of  (Your Loved One)", "Student Name, Class of 2010", or perhaps just a family or business name.
